[10/12] lpfc: Remove Menlo/Hornet related code

Series lpfc: Update lpfc to revision 14.2.0.5 | expand

The Menlo/Hornet adapter was never released to the field. As such,
driver code specific to the adapter is unnecessary and should be
removed.
